Will Gynecomastia Go Away on Its Own?

Sometimes, but usually not. If you are a teenager going through puberty, there is a good chance the excess breast tissue will shrink on its own within a year or two. But if you are an adult man over 20 and you have had enlarged breasts for more than two years, the tissue has likely become permanent. No amount of chest exercises, dieting, or fat burners will make true glandular breast tissue disappear. A 35-year-old who has been doing 200 push-ups a day for three years and still has a soft, rounded chest is not going to fix this at the gym. That is glandular tissue, not fat. What Is Gynecomastia?

Gynecomastia is the medical term for enlarged male breast tissue. It is not just excess fat. True gynecomastia involves actual breast gland tissue that develops behind the nipple and areola area. This tissue is firm, sometimes tender, and cannot be eliminated through exercise or diet.

There is also a related condition called pseudogynecomastia, which is simply excess fat in the chest area without glandular tissue. The distinction matters because pseudogynecomastia can sometimes improve with weight loss or liposculpting, while true gynecomastia requires surgical removal of the glands.

During your consultation, I examine your chest to determine whether you have true gynecomastia, pseudogynecomastia, or a combination of both. This determines the surgical approach. Most situations that I encounter is the combination of both.

  • Hormonal changes during puberty that do not fully resolve
  • Hormonal imbalances in adult men (elevated estrogen, low testosterone)
  • Medications including steroids, anti-anxiety drugs, heart medications, and certain antidepressants
  • Marijuana and alcohol use
  • Weight gain and obesity (contributes to pseudogynecomastia)
  • Aging and natural testosterone decline
  • Unknown causes (many cases are idiopathic)

  • Bodybuilders or fitness-focused men who have developed gynecomastia from anabolic steroid use (current steroid use must be stopped before surgery)
  • Men with asymmetrical breasts where one side is noticeably larger
  • Teen males (18+) whose gynecomastia has persisted for more than two years past puberty

If you are significantly overweight, I may recommend losing weight first. Removing glandular tissue while there is still a large amount of surrounding fat will not give you the best result. In some cases, I recommend our medical weight loss program as a first step, then surgery once you are closer to your goal weight.

What to Expect During Gynecomastia Surgery

Consultation

We start with a thorough examination. I check your chest tissue to determine whether you have true gynecomastia, pseudogynecomastia, or a combination. I assess the amount of glandular tissue, fat, and skin excess. I also review your health history and any medications you are taking (some medications cause gynecomastia, and changing them may be part of the plan). Consultations are free and completely private.

The Procedure

Gynecomastia surgery is performed at our AAAHC-accredited facility in Costa Mesa under general anesthesia or IV sedation depending on the extent of the procedure. There are two main techniques I use, often in combination:

Liposuction: For excess fat, I use small incisions (3mm) placed in discreet locations and suction out the fat using a thin cannula. This works well for pseudogynecomastia and the fatty component of true gynecomastia.

Direct tissue excision: For firm glandular tissue behind the nipple that liposuction cannot remove, I make a small incision along the lower border of the areola and surgically remove the tissue. This incision heals well and is nearly invisible once mature.

In more severe cases with significant skin excess, additional incisions may be needed to remove loose skin and reposition the nipple. Surgery typically takes 1 to 2 hours.

After Surgery

You go home the same day. You will wear a compression vest for 4 to 6 weeks to support healing and help the skin contract smoothly. Someone needs to drive you home.

Recovery and Downtime After Gynecomastia Surgery

Days 1 to 3: Soreness, swelling, and some bruising. Pain is manageable with prescribed medication. Keep the compression vest on.

Days 3 to 5: Most patients return to desk work and light daily activities.

Weeks 2 to 4: Swelling continues to decrease. You can gradually increase activity. Continue wearing the compression vest as directed.

Week 6: You can resume full exercise, including chest workouts.

Months 3 to 6: Final results become fully visible as all residual swelling resolves. Incision lines continue to fade.

How Much Does Gynecomastia Surgery Cost in Orange County?

Why Surgeon Experience Matters for Gynecomastia

Gynecomastia surgery looks simple on paper: remove the tissue, contour the chest. But the difference between a good result and a bad one comes down to how much tissue the surgeon removes and where they leave it.

Remove too much tissue directly behind the nipple and you get a crater deformity, a visible dent in the chest that looks worse than the gynecomastia did. Remove too little and the patient is disappointed because the puffiness is still there. The margin is small, and it takes experience to get it right consistently.

I also pay close attention to feathering, which means blending the transition between the treated area and the surrounding chest so there is no visible edge or step-off. That is what makes a result look natural rather than surgical.

What Most Patients Don't Realize About Gynecomastia Surgery

The number one thing men underestimate about this procedure is the emotional impact. I have had grown men tear up in the consultation room just talking about how gynecomastia has affected their life. And I have had patients come back for their one-week follow-up grinning because they now can wear a fitted t-shirt in public for the first time in years.

The other thing patients do not realize is that not all gynecomastia is the same. A man with soft, fatty chest fullness needs a completely different surgical plan than a man with a hard disc of glandular tissue behind each nipple. And a man with significant skin laxity on top of everything else needs a third approach entirely. If a surgeon offers you the same operation regardless of what your chest looks like, that is a warning sign.

I also see patients who tried non-surgical alternatives first. CoolSculpting, chest exercises, hormone supplements, compression shirts. None of those address glandular tissue. If you have true gynecomastia, surgery is the only permanent solution. I would rather tell you that honestly than have you waste time and money on treatments that will not work.

Gynecomastia vs. Chest Fat: How to Tell the Difference

This is one of the most common questions I get. Here is a simple way to check at home:

Stand in front of a mirror and pinch the tissue around your nipple between your thumb and forefinger. If you feel a firm, rubbery disc of tissue behind the nipple (almost like a small hockey puck) that also hurt, that is glandular tissue, which means true gynecomastia. If the tissue is uniformly soft and fatty with no firm disc, you likely have pseudogynecomastia (excess chest fat).  This is also painless.

Will gynecomastia come back after surgery?

In the vast majority of cases, no. Once glandular tissue is surgically removed, it does not grow back. However, if gynecomastia was caused by medication or steroid use and you resume those, there is a small chance of recurrence in any remaining tissue. Significant weight gain can also increase chest fat. I discuss all of this during your consultation so you know exactly what to expect long-term.
