Who Is an Ideal Candidate for a Brazilian Butt Lift (BBL)?

A good BBL candidate is someone in general good health who has enough donor fat for transfer and wants a fuller, more proportional shape to their buttocks. You do not need to be a specific weight. I have performed BBLs on patients ranging from a size 4 to a size 16. What matters is that you have enough fat in areas like the abdomen, flanks, or thighs for us to harvest, purify, and transfer. If you can pinch at least an inch in those areas, we can usually work with it. Not all areas can be used for fat harvesting.

The patients who get the best results are the ones who come in with a clear idea of proportion, not just size. "I want it bigger" is a starting point, but "I want my waist to look smaller and my hips to have a rounder shape" gives me something real to work with. What Is a Brazilian Butt Lift (BBL)?

A Brazilian Butt Lift uses your own body fat to add volume and shape to your buttocks. There are no implants involved.

Here is how I perform the procedure:

Step 1: Fat Harvesting

I use Vibro-Tumescent fat harvesting to remove fat from areas where you have excess, typically the abdomen, flanks, lower back, or thighs. This is not standard liposuction. The vibro-tumescent technique I use breaks up fat more gently, which means less bruising for you and healthier fat cells for transfer. Remember its “Fat Harvesting”, not “Liposuctioning”.

Step 2: Fat Purification

The harvested fat is processed and purified. Not all fat cells survive transfer, so the quality of processing directly affects your long-term results. I am selective about what gets transferred because packing in low-quality fat cells just to add volume leads to poor retention and lumpy results down the road.

Step 3: Fat Grafting

I inject the purified fat into your buttocks at multiple depths and angles. This is the part that separates experienced surgeons from everyone else. Placement determines shape, and shape is everything.

The entire procedure takes roughly 2 to 3 hours and is performed under general anesthesia in our AAAHC-accredited surgical facility in Costa Mesa.

Who Is a Good Candidate for BBL Surgery?

I see BBL patients from all walks of life. Here are some common situations:

You have always had a flat or square-shaped backside, and no amount of squats has changed that. Genetics play a bigger role in buttock shape than most people realize.

You lost a significant amount of weight and your buttocks deflated along with everything else. You are happy with your weight loss but want your proportions back.

You had children and your body shifted. Your midsection expanded and your backside flattened. A BBL combined with liposuction can restore balance.

You have realistic expectations. This is probably the most important one. A BBL can give you a noticeable improvement in shape and volume, but it cannot replicate what you see on heavily filtered social media posts.

Who is NOT a good candidate: If you have very low body fat and minimal donor areas, a BBL may not be the best option. I will tell you that during the consultation rather than setting you up for a disappointing result.

What to Expect During Your BBL Procedure

Before surgery

You will come in for a consultation where I examine your body, discuss your goals, and determine whether you have enough donor fat. I will show you before-and-after photos of patients with similar body types. If we both agree you are a good candidate, we schedule the procedure.

Day of surgery

Plan for roughly 2 to 3 hours in the operating room.  The first portion is fat harvesting. The second is fat processing and injection.

Immediately after

You will go home the same day. You will need someone to drive you and someone to stay with you for the first 24 hours whom is over 18 years of age. You will be in a compression garment and will need to avoid sitting directly on your buttocks for about two weeks.

The first week

Soreness is real but manageable. Most patients describe it as feeling like a deep muscle ache, similar to what you would feel after an intense lower-body workout. I prescribe medication to keep you comfortable.

Recovery and Downtime After BBL Surgery

Week 1: The hardest part. You need to sleep on your stomach. Sitting is restricted to sitting straight up. Swelling and bruising are normal. Take the 3-4 days off work.

Weeks 2 to 3: Most patients can return to a desk job if they use a BBL pillow to keep pressure off the buttocks. Swelling is going down. You are starting to see early results, but do not judge the final shape yet.

Weeks 4 to 6: You can gradually resume light exercise. Walking is encouraged throughout recovery, but no heavy lifting or squats until I clear you. Your body is still healing and the transferred fat is establishing blood supply.

Month 3: This is when you start seeing a realistic preview of your final shape. Most swelling is gone.

Month 6 to 12: Final results. The fat cells that survived transfer are now permanent. Typically 60% to 80% of the transferred fat integrates permanently if its done correctly and not over injecting to restrict blood supply distribution.

A lesson I share with every patient: Do not panic at Week 2 when you feel like you are losing volume. Some fat absorption is normal and expected. I account for this during the procedure by slightly overfilling. Patients who try to judge their results too early end up unnecessarily stressed.

Why Surgeon Experience Matters for BBL

The BBL has one of the highest complication rates of any cosmetic procedure when performed by inexperienced surgeons. The primary risk is fat embolism, where fat enters the bloodstream and travels to the lungs. This is almost entirely preventable with proper technique.

Here is what I do differently:

I inject fat into the subcutaneous layer only, never into or below the muscle. Injecting into the gluteal muscle is what causes fat emboli. This is now the standard recommendation from every major cosmetic surgery society, but not every surgeon follows it.

I use a 4mm cannula to inject the fat which is a bigger size and hard to penetrate in the blood vessels.

I inject along the anatomical position of the muscle which prevents me from going into the muscle that increases the risk of fat emboli.

I operate in an AAAHC-accredited facility with board-certified anesthesiologists. Not nurse anesthetists, not sedation-only setups.

Your surgeon's experience is not just about how many BBLs they have done. It is about whether they have done enough to know when to say no, when to adjust the plan, and how to handle the unexpected.

What Most Patients Do Not Realize About BBL Surgery

Most patients focus on "how big" when they should be focusing on "what shape." I can make your buttocks larger. Any competent surgeon can add volume. But creating a shape that looks proportional to your frame, that looks natural from every angle, that actually fits your body, requires a different level of judgment.

Another thing patients overlook: when liposuction is added to the BBL is half the procedure.  Sculpting the waist, flanks, and lower back is what creates the visual contrast that makes your buttocks look fuller. Some of the most dramatic BBL results I have achieved were cases where the liposuction contouring did most of the heavy lifting.

And finally, your weight after surgery matters more than you think. Gaining 15 or 20 pounds after a BBL changes your results because the transferred fat cells behave just like normal fat. They grow when you gain weight and shrink when you lose it. Maintaining a stable weight is the single best thing you can do to protect your investment.
